Life Insurance: Change Beneficiary Protocol

Change Beneficiary

To Change Beneficiary, please download the
Multi-Purpose Change form and complete the following steps:
Please complete the Policy Owner Information section for all change requests.
  • Complete all portions of the Change Beneficiary section. If multiple beneficiaries are listed, please designate the amount of the benefit in percentages. Ensure the percentage equals 100%.
  • If a group is named as beneficiary, you must name each individual of this group. If a minor (below the age of 18) is listed as a beneficiary, please appointment of a trustee as financial guardianship for the minor will be required before policy proceeds can be released.
  • Be sure to also complete and sign the Authorization for Beneficiary Change section of the form.
  • If there is an existing Irrevocable Beneficiary assigned to your policy, we need their signature to process the change request.
  • Mail or fax the Multi-Purpose Change Form to:
